Identify exactly what was approved

A screening result, Medicaid eligibility notice, and personal care service authorization are not interchangeable. Ask which document you have, what service is approved, and which dates apply. If the paperwork is unclear, call us and explain what you received.

Choose a provider able to meet the needs

Ask whether the agency participates with the relevant coverage, serves the address, and can support the care needs and proposed schedule. Discuss nurse oversight, communication, and what happens when there is a change in condition.

Review the care plan and caregiver

The agency evaluates the care arrangement and confirms the necessary steps. If a family member hopes to provide the care, their qualifications and employment requirements must also be reviewed. An authorization for the member is not an employment offer to a relative.

Confirm the start and ongoing responsibilities

Make sure the start date, schedule, contact process, and visit-recording instructions are clear. Ask how to report hospital admissions, missed visits, or changing needs. Keep track of authorization dates and respond when the provider or plan requests information.

Do not assume unapproved additional hours are covered. If needs change, request review through the agency and relevant care coordinator.
